Central Language
I understand that App
Central is a product of CACTUS Software that has been purchased and is utilized
by MERCY HEALTH for the purpose of collecting and communicating information
necessary to complete the credentialing and privileging process for MERCY
HEALTH. The information requested
through App Central is only used for the credentialing and privileging
processes associated with MERCY HEALTH.
ID and Password
I understand that I will create a unique
identification (ID) code and password to be used to access App Central. I understand that this ID and password are
unique codes that identify me in the App Central system. Information and entries that I make via App
Central will be logged with my identity.
I understand that it is extremely important that
I keep the ID and password that I use to access App Central confidential. If at any time I feel that the
confidentiality of my password has been compromised, I will change it by going
through the process identified in App Central.
I understand that MERCY HEALTH takes no
responsibility for and disclaim any and all liability or consequential damages
arising from a breach of confidentiality resulting from my sharing or losing my
password.
If the staff of MERCY HEALTH discovers that I
have inappropriately shared my password with another person, or that I have
misused or abused my access with App Central, my continued access to App
Central may be discontinued by the healthcare team without prior notice.
Communication
received through App Central
I understand that communication with the
Credentialing/Medical Staff Services staff will be completed through App
Central, email communication external to App Central, i.e., Groupwise,
and through direct conversation (in person, by telephone). The anticipated turnaround time for response
to electronic messages is 1 to 2 business days.
Integrity
and Validity of Information Submitted through App Central
I understand that all information submitted for
the credentialing and privileging process, either through App Central or though
any other means, is true, complete, and correct. I agree to supplement the
information in my application and supporting documents should any statement,
although true when made, may become untrue due to change or discovery of new
information. I also understand that the
documents completed in App Central may also contain further attestations and
statements that call for me to validate that the information is true, complete,
and correct.
Use
of an Electronic Signature - IMPORTANT
I understand that an electronic signature can be
used with any document provided through App Central including the
application.
I hereby agree that my typed name on a signature
line on any document provided through App Central is the same as and equates to
my written signature. I understand that
this electronic signature represents my written signature and is no less
official or binding as a written signature.

of Computer and Network Security
You are attempting
to obtain access to a component of a computer resource that has been purchased
by MERCY HEALTH. These resources,
including all related equipment, networks, and network devices (specifically
including Internet access), are provided only for authorized use. All data residing on any component of this
system is the property of the State of ARKANSAS, MISSOURI, KANSAS and OKLHAOMA.
MERCY HEALTH
computer resources may be monitored for all lawful purposes, including ensuring
that their use is authorized, for system management, to facilitate protection
against unauthorized access, and to verify security procedures, survivability,
and operational security. Monitoring
includes security testing by authorized entities to test or verify the security
of this system. All information,
including personal information, placed on or sent over this system may be used
for authorized purposes only.
Unauthorized use of
App Central may subject you to criminal prosecution. Evidence of unauthorized use may be used for
administrative, criminal or other disciplinary action. Use of this computer system, authorized or
unauthorized, constitutes consent to monitoring of this system.
Disclaimer
I understand that MERCY HEALTH may not be
available to me all the time due to system failures, back-up procedures,
maintenance, or other causes beyond the control of MERCY HEALTH. Access is provided on an "as-is,
as-available" basis and does not guarantee that I will be able to access
App Central at any particular time.
I UNDERSTAND THAT MERCY HEALTH TAKES NO
RESPONSIBILITY FOR AND DISCLAIM ANY AND ALL LIABILITY ARISING FROM ANY
INACCURACIES OR DEFECTS IN SOFTWARE, COMMUNICATION LINES, VIRTUAL PRIVATE
NETWORK, THE INTERNET OR MY INTERNET SERVICE PROVIDER (ISP), ACCESS SYSTEM,
COMPUTER HARDWARE OR SOFTWARE, OR ANY OTHER SERVICE OR DEVICE THAT I USE TO
ACCESS MERCY HEALTH MYCHART.
During times when App Central is unavailable, other communication methods
(e.g., telephone) should be used to access the Credentialing/Medical Staff
Services Department at MERCY HEALTH.
